Attendance and Absence Reporting

To report your child absent:

Email Sixth Form LS6Attendance@leafstudio.co.uk 

confirming your child’s name, year group and a brief description of the reason for absence. 

To request absences (in advance):

Complete this form for any form of pre-planned absence

Complete this form for weekly recurring sporting or performing arts absences

- Pre-planned absence (not exhaustive) = holiday, doctor/dentist, funeral, one off performing arts exam, one off training session, interview, open day, performance contract

- Weekly recurring absence (not exhaustive) = a training class that takes place on a weekly basis
